Step 1: Gather Documents
Examples of Additional Supporting Documents
If applicable, additional information may be requested. Some examples:

Proof of death such as death certificate, obituary, funeral program, medical examiner report or notes.

A physician’s letter with letterhead that includes diagnosis, prognosis and return to work date. Wings will accept legal documents that has been or will be submitted to American Airlines Absence and Return Center, (ARC)

Documentation provided by APFA EAP or American Airlines EAP.

Outstanding medical bills with explanation of claim due to illness or injury.

Proof of accident or incident such as police report or insurance claim. This is for bodily injury (personal self). Wings will not approve a grant for vehicle, motorcycle or RV coverage. This includes but not limited to vehicle insurance deductible or damage, rental car and ride share charges.

Outstanding medical bills with explanation of claim due to illness or injury.

Tax returns (1040).

All banking account statements held in applicants name (joint or solo).

Explanation of circumstances related to hardship.
